The 2015 ISSVD-NVA-IPPS consensus terminology defines vulvodynia as pain of the vulva that is idiopathic by definition, meaning every visible cause must be ruled out first. Underneath that "idiopathic" label are real, identifiable contributors: pelvic floor dysfunction, central sensitization of pain neurons, mast cell activation, low estrogen states, gut and vaginal microbiome imbalance, and a long history of chronic vulvar irritation. Non-burning sensory abnormalities, itching that does not respond to antifungals, tingling, "crawling skin", and electric-shock sensations are common, especially in generalized vulvodynia. Reflect involvement of small nerve fibers and abnormal nerve firing.
Many women report needing to sit on a special cushion, avoiding jeans, and avoiding certain chairs at work. Long drives, flights, bicycle riding, and tight underwear become unbearable. This is often the first symptom that affects daily functioning.
