What Is Appendicitis?

Appendicitis is acute inflammation of the vermiform appendix, a finger-like pouch attached to the cecum at the start of the colon. It typically results from obstruction of the appendiceal lumen by a fecalith (hardened stool), lymphoid hyperplasia, or rarely a tumor, leading to bacterial overgrowth, inflammation, and potential rupture.

Untreated, appendicitis progresses from inflammation (24-48 hours) to gangrene to perforation with peritonitis, abscess formation, and potentially sepsis. Perforation rates are ~20-30% by 36 hours from symptom onset.2 This is a SURGICAL EMERGENCY, time matters significantly.

Lifetime risk is ~7-8%, with peak incidence in adolescents and young adults (10-30 years).1 Children and elderly often present atypically and have higher complication rates. Modern management is increasingly individualized, uncomplicated appendicitis may sometimes be treated with antibiotics alone, but surgery remains the gold standard for definitive treatment.

Time Is Everything

Active appendicitis is a true surgical emergency, every hour of delay increases risk of perforation, peritonitis, and serious complications. Don't try home remedies for suspected appendicitis. Once you're recovered from surgery, focus on nutrition for healing, microbiome restoration after antibiotics, and high-fiber prevention strategies.

Appendicitis is a surgical emergency, and it is the one condition on this site where using nutrition information at the wrong moment can cost you an organ or a life. If you have right lower abdominal pain, particularly with fever, nausea or pain that started around the navel and moved, go to an emergency department now. Do not eat or drink, because you may need an anaesthetic. Do not take painkillers, because they mask the sign clinicians examine for. Do not wait for the pain to settle, because pain often eases in the hours before perforation, and that relief is a warning rather than a reassurance. Nothing on this page treats appendicitis. Everything here is either about recovering well after the operation or about general bowel health, and none of it is a reason to delay by even an hour.

  1. On epidemiology: lifetime risk of appendicitis is commonly cited at around 7 to 8%, PubMed 22948195, with peak incidence between roughly 10 and 30 years of age. Appendicitis is the most common abdominal surgical emergency. Presentation is frequently atypical in young children, older adults and pregnancy, which is the reason those groups have higher perforation rates at diagnosis.
  2. On progression and timing: untreated appendicitis progresses from mucosal inflammation through gangrene to perforation, with peritonitis or abscess formation. Reported perforation rates rise steeply with time from symptom onset, and series commonly report substantial perforation by around 36 hours. The exact percentages vary between series and populations; the direction does not. Delay is the modifiable risk.
  3. On diagnosis: contrast-enhanced CT has a summary sensitivity of about 96% in adults and a specificity of roughly 93 to 95%, PubMed 31743429, with an appendiceal diameter over 6 mm, wall thickening and peri-appendiceal fat stranding as the key findings. Ultrasound is preferred first in children and pregnancy to avoid radiation. A normal white cell count does not exclude appendicitis, and around 10% of patients have one, which is why imaging and serial examination matter more than a single blood test. A pregnancy test is mandatory in any woman of childbearing age with abdominal pain, to exclude ectopic pregnancy.
  4. On appendectomy: laparoscopic appendectomy is the standard approach for most patients, with less pain, shorter stay and fewer wound infections than open surgery. Surgical site infection after laparoscopic appendectomy for uncomplicated disease is generally reported in the low single digits, around 1 to 2%, and is higher after open surgery or when the appendix has perforated. Intra-abdominal abscess is the complication more associated with perforated disease.
  5. CODA Collaborative. A randomized trial comparing antibiotics with appendectomy for appendicitis. N Engl J Med. 2020;383:1907–1919. nejm.org. 1,552 adults randomized. Antibiotics were non-inferior to appendectomy on 30-day health status. 29% of the antibiotics group had undergone appendectomy by 90 days. Longer follow-up from the same cohort reports roughly 40% by one year and about half by four years. The 90-day figure is the one usually quoted, and quoting it without the time window makes antibiotics look more durable than they are.
  6. On who is not a good candidate for antibiotics first: in CODA, patients with an appendicolith (fecalith) had a markedly higher rate of appendectomy, around 41% by 90 days, and a higher rate of complications. (CODA Collaborative. N Engl J Med. 2020;383(20):1907–1919, in which 414 of 1,552 randomised adults had an appendicolith. PubMed 33017106) Perforation, abscess, generalised peritonitis and suspected tumour are not situations for antibiotic-only management. This is a decision to make with a surgeon who has seen the imaging, not one to make from a web page.
  7. On fiber and prevention. The idea that low dietary fiber causes appendicitis comes from Burkitt's mid-twentieth-century comparisons of populations with different diets, in which appendicitis was one of several conditions grouped under a single low-fibre explanation, PubMed 28583217, supported since by observational associations. It has never been tested in a randomized trial, and it remains a hypothesis rather than an established cause. Fiber is worth eating for bowel health, cholesterol and colorectal cancer risk, all of which have better evidence behind them. Preventing appendicitis is not a claim this site can make for it.
  8. On recovery after surgery: adequate protein, vitamin C and zinc support wound healing, and correcting a deficiency matters more than supplementing someone already replete. Saccharomyces boulardii and multi-strain probiotics have reasonable randomized evidence for reducing antibiotic-associated diarrhoea, which is the specific reason they appear on this page. Early mobilisation and a normal diet as tolerated are standard modern practice; prolonged bowel rest is not.
