Appendicitis is acute inflammation of the vermiform appendix, a finger-like pouch attached to the cecum at the start of the colon. It typically results from obstruction of the appendiceal lumen by a fecalith (hardened stool), lymphoid hyperplasia, or rarely a tumor, leading to bacterial overgrowth, inflammation, and potential rupture.
Untreated, appendicitis progresses from inflammation (24-48 hours) to gangrene to perforation with peritonitis, abscess formation, and potentially sepsis. Perforation rates are ~20-30% by 36 hours from symptom onset. This is a SURGICAL EMERGENCY, time matters significantly.
Lifetime risk is ~7-8%, with peak incidence in adolescents and young adults (10-30 years). Children and elderly often present atypically and have higher complication rates. Modern management is increasingly individualized, uncomplicated appendicitis may sometimes be treated with antibiotics alone, but surgery remains the gold standard for definitive treatment.
