What Is Celiac Disease?

Celiac disease is a chronic autoimmune disorder triggered by gluten (a protein in wheat, barley, and rye) in genetically susceptible individuals. When gluten is consumed, the immune system mounts an attack on the small intestine's villi (finger-like projections that absorb nutrients), leading to villous atrophy, malabsorption, and a cascade of nutritional deficiencies.

Approximately 1% of the global population has celiac, but ~80% remain undiagnosed. Symptoms vary dramatically, from classic GI complaints to subtle, "silent" presentations dominated by fatigue, anemia, infertility, neurological symptoms, or skin rash (dermatitis herpetiformis3). Many adults are diagnosed only after years or decades of vague symptoms.

Two specific genes, HLA-DQ2 and HLA-DQ8, are necessary (but not sufficient) for celiac. ~30% of the population carries these genes but only ~3% of carriers actually develop celiac2. The trigger is the interplay between genetics, gluten exposure, gut microbiome, and intestinal permeability (leaky gut).

โš ๏ธ Celiac is ~10x more common in first-degree relatives of celiac patients4, and ~5x more common in patients with other autoimmune diseases (type 1 diabetes, Hashimoto's, autoimmune liver disease). Anyone in these groups should be screened, even without symptoms.

Forms of Celiac Disease

GI-dominant: chronic diarrhea, weight loss, abdominal pain, malabsorption. Most often diagnosed in children. Now actually a minority of adult diagnoses.

๐ŸŒ— Non-Classical (Silent) Celiac

No classic GI symptoms but extra-intestinal manifestations dominate: iron-deficiency anemia, osteoporosis, infertility, neurological symptoms, dermatitis herpetiformis. Now the majority of adult diagnoses.

NCGS, distinct from celiac. Symptoms with gluten but no antibodies, no villous atrophy. Mechanism: possibly FODMAP, ATIs (amylase trypsin inhibitors), or leaky gut. Prevalence is genuinely uncertain, with estimates ranging from under 1% to around 13%, because there is no diagnostic test and it is defined by exclusion.8 Doesn't carry same long-term risks but warrants gluten avoidance for symptom control.

  • STRICT gluten-free diet, and the amount genuinely matters. Damage has been demonstrated at sustained intakes around 50 mg of gluten per day, about a crumb of bread, while 10 mg a day produced essentially none in the trial that set the threshold. The 20 ppm limit for gluten-free labeling exists precisely to keep daily intake below that, so certified gluten-free food is not the problem.6 Crumbs, shared toasters, restaurant cross-contamination all matter. Coeliac antibodies fall and the intestinal lining begins to recover once gluten is removed, so testing afterwards can be falsely negative and the diagnosis becomes genuinely hard to establish. Going back on gluten later, a gluten challenge, means weeks of deliberately eating the thing that harms you, and many people cannot face it. So if you suspect coeliac disease, keep eating gluten and ask for the blood test now. Unlike most pages on this site, the treatment here is dietary and it is the whole treatment: a strict lifelong gluten-free diet is not an adjunct to something else, it is the medicine.

  1. On diagnosis: serological testing with tissue transglutaminase IgA (tTG-IgA) alongside total IgA is the standard first step, followed by duodenal biopsy in adults to confirm villous atrophy. Both must be performed while the person is still eating gluten. Total IgA is measured at the same time because selective IgA deficiency is more common in coeliac disease than in the general population and produces a false negative tTG-IgA; in that situation IgG-based tests are used instead.
  2. On genetics: HLA-DQ2 and HLA-DQ8 are necessary but not sufficient. Roughly 30% of the general population is commonly reported to carry one of them and only a small minority of carriers develop coeliac disease, so a positive genetic test does not diagnose anything. Its value is the reverse: a negative result makes coeliac disease very unlikely, which is useful for ruling out, particularly in someone already gluten-free.
  3. On presentation: classical malabsorptive presentation with diarrhoea and weight loss is now the minority in adults. Iron-deficiency anaemia, osteoporosis, raised transaminases, infertility, recurrent miscarriage, peripheral neuropathy, ataxia and dermatitis herpetiformis are common presenting features, and coeliac disease is frequently found during investigation of those rather than of bowel symptoms. Dermatitis herpetiformis is effectively coeliac disease of the skin and responds to the same diet.
  4. On screening family members. First-degree relatives of someone with coeliac disease have a substantially higher risk than the general population, commonly cited around 1 in 10, and many are asymptomatic or have only extra-intestinal signs. Testing relatives is recommended, and it is one of the more useful things a newly diagnosed person can pass on.
  5. On the spleen and vaccination. Coeliac disease is associated with functional hyposplenism, meaning the spleen works less well than it should, which raises susceptibility to encapsulated bacteria. Pneumococcal vaccination is recommended for people with coeliac disease in several national guidelines. This is easy to miss because it has nothing to do with diet, and it is worth raising with your own clinician.
  6. On how much gluten is too much, and what 20 ppm actually means. The trial that established the threshold gave people with coeliac disease 0, 10 or 50 mg of gluten daily for three months: the 50 mg group showed measurable change in the intestinal lining, the 10 mg group showed essentially none, and the authors concluded that contaminating gluten should be kept below 50 mg a day, PubMed 17209192. Fifty milligrams is roughly a crumb of bread. The 20 parts per million threshold is a labelling standard, not a damage threshold. It is the ceiling below which a food may be sold as gluten-free, and it was chosen so that a normal day's eating of such foods stays under the harmful daily intake. Certified gluten-free products are therefore part of the solution. The uncontrolled exposures are what matter: shared toasters, fryer oil, bulk bins, floured surfaces, and restaurant kitchens.
  7. On long-term risk, stated carefully. Untreated or persistently active coeliac disease carries an increased relative risk of enteropathy-associated T-cell lymphoma and small bowel adenocarcinoma. The absolute risk remains low, these are uncommon cancers, and the great majority of people with coeliac disease never develop them. A US cohort found raised risks of small intestinal adenocarcinoma and non-Hodgkin lymphoma, and reported that the lymphoma risk persisted despite a gluten-free diet, PubMed 12935825. Risk is generally understood to fall with sustained mucosal healing, but that one finding is worth knowing rather than hiding. The reason to include this is not alarm but motivation: it is the honest answer to why the diet needs to be exact rather than approximate, and why a persistently raised tTG deserves investigation rather than resignation.
  8. On non-coeliac gluten sensitivity: NCGS describes symptoms attributed to gluten in people without coeliac serology, villous atrophy, or wheat allergy. There is no biomarker, and it is diagnosed by exclusion after coeliac disease has been ruled out, which is the order that matters. Reported prevalence varies by an order of magnitude between studies for exactly that reason. Some of the response is likely attributable to fructans and other FODMAPs in wheat rather than to gluten itself. It does not carry the same long-term risks as coeliac disease.
